Nurse Nav Advocate supports your ability to be in control. Having someone who understands what good care looks like insisting you receive it and get treated in active partnership with your providers is needed.  Giving you and your family peace of mind.  Especially when, for some reason, they are unable to support right now.  Nurse Nav can anticipate problems and discretely and calmly ensure you get safe, quality, comprehensive care.

Sometimes you need support from someone who can step in and help you get what you need, especially when you are unwell and vulnerable. Nurse Nav can help you or a loved one get what they need from a health service.

What to expect:

As part of this Package we have quite a long (up to 2.5 hours) initial assessment of your concerns and condition with your family if you want them to be included in the discussion. This part is really important.  We also gather any health information we need in the form of your history, previous tests and treatments, the people you have on your team now, what you would like to happen and goals of care.

From there together we figure out what you need, want and prefer. Nurse Nav will support, guide and advise letting you know what to reasonably expect, what is currently available including government funded services you may not know about. We work for you and will work hard to get you what you need.

Our expert eye will review documents, explain discharge summaries, explain everything you need explaining, and be the go-between to other services, improve communication and collaboration between stakeholders including ensuring your GP has all the information they need to support you in making decisions about your care.

We can then advocate on your behalf, or support a family member/carer or friend to do it if that makes you feel more comfortable.

Value of the package is approx 3 sessions: Depending on the length of time of initial assessment up to 2.5 hours, then the other sessions are what is required.  We can meet using a variety of options: phone, facetime, zoom, skype, or if you are local, face to face.

We will provide you and your team with a plan of care that describes your goals and what everyone is responsible for.
